If you’ve started your private practice after working in the NHS or another clinical setting, chances are your business is built around one-to-one client sessions.

It’s the most natural place to start.

Your training taught you to deliver care directly to individuals. Your workday was structured around appointments. You assessed, advised and supported one patient at a time. So when you launch your own business, it makes sense that you replicate that model.

But what works in a salaried role doesn’t always translate well into running a business.

In the NHS, your salary didn’t depend on the number of patients you saw. In private practice, every session directly affects your income. And that’s where the limitation of the 1:1 model becomes clear.

At first, filling your diary feels like success. But eventually many practitioners discover something uncomfortable.

You can be fully booked and still feel stuck.

Your income is capped by the number of hours you can physically work.

That’s when it becomes important to start thinking about how to move beyond 1:1 clients and introduce more scalable ways to deliver your expertise.

 

The Hidden Ceiling of the 1:1 Model

When your business relies entirely on client sessions, three things happen.

  1. Your income becomes directly tied to your time. If you want to earn more, you have to work more hours.
  2. Your energy becomes your most limited resource. Back to back consultations can be rewarding, but they’re also demanding.
  3. Your impact is limited. You can only help as many people as you have appointment slots.

Many dietitians don’t realise they’ve hit this ceiling until they feel overwhelmed, exhausted or frustrated that their income isn’t growing.

So how do you know when it’s time to move beyond 1:1 clients?

Here are five clear signs.

 

5 Signs It’s Time to Move Beyond 1:1 Clients

 

1. Your Diary Is Full but Your Income Feels Capped

If you’re consistently booked but still not earning the income you want, this is often the first indicator.

Ask yourself:

  • Are you fully booked most weeks?

  • Would doubling your income mean doubling your client hours?

  • Is there no room left in your schedule without sacrificing personal time?

If the answer is yes, your current model has likely reached its natural limit.

 

2. You’re Repeating The Same Advice

This is actually a very positive sign.

When you find yourself explaining the same concepts again and again, it means your knowledge is systematic and teachable.

Patterns start to emerge:

  • Clients ask the same questions

  • They face similar challenges

  • You guide them through similar steps

Anything you say repeatedly can be taught once and delivered many times.

That’s the foundation of scalable offers like courses, programmes or group coaching.

 

3. Your Clients Need You Less Over Time

When you’re good at what you do, your clients stabilise faster.

They gain confidence. They implement your advice. They begin managing things independently.

Instead of seeing this as a reason to hold onto 1:1 work, it can be a signal that your method works well enough to be delivered in other formats.

Many clients don’t actually need regular one-to-one support. But they do need:

  • Clear guidance

  • Accountability
  • Structured steps

  • Occasional expert input

Those needs can often be met through group programmes or structured courses.

 

4. You’re Turning People Away

Another common sign appears when your schedule becomes too full.

You might notice:

  • A waiting list forming

  • Enquiries you can’t accommodate

  • Clients wanting to work with you but no available appointments

At that point, your current model is limiting both your income and your impact.

 

5. Your Personal Life Is Colliding With Your Clinic Hours

Finally, there’s the energy factor.

Back-to-back client sessions can leave you mentally drained. By the end of the day, you may have very little left for family, hobbies or rest.

If your evenings and weekends are spent recovering from the working week, your business model may need redesigning.

 

What Moving Beyond 1:1 Clients Actually Looks Like

Moving beyond 1:1 clients doesn’t mean abandoning personal support.

One of the most sustainable approaches is to keep a limited number of premium 1:1 clients while introducing one leveraged layer into your business.

This could include:

  • Online courses

  • Group programmes

  • Memberships

  • Workshops

  • Digital resources

The goal is not to replace your expertise but to deliver it in ways that help more people without increasing your working hours.

 

A Simple Tiered Model That Works

A helpful way to think about this is by creating different levels of support.

For example, in my own children’s nutritionist business I created three tiers within my programme Fussy Eating Fixed.

Self Study Option

The entry level option gives parents access to the course and community. They work through the material independently and implement the guidance at their own pace.

This works well for people who want results but don’t need direct support.

 

Supported Programme

The next level includes access to the course and community plus additional expert support through group sessions and Q&A opportunities.

This provides reassurance and guidance without requiring constant one-to-one contact.

 

VIP One-to-One Support

The premium option includes personalised one-to-one support, assessments and tailored plans.

Because this level requires significant time and expertise, places are limited and priced accordingly.

This structure allows clients to choose the level of support that suits them while allowing the business owner to manage time and energy sustainably.

 

Why This Model Works

This tiered approach offers several advantages.

First, it allows you to help more people because not every client requires one-to-one attention.

Second, it protects your energy by reducing the number of back-to-back sessions.

Third, it creates multiple income streams, which makes your business more resilient.

And importantly, it still preserves personalised support for the clients who need it most.

 

How to Start Moving Beyond 1:1 Clients

If you’re considering making this shift, you don’t need to redesign your entire business overnight.

Instead, start with clarity.

Ask yourself:

  • Am I repeating the same advice to clients?

  • Is my income limited by the number of hours I can work?

  • Does my business rely entirely on my availability?

If the answer is yes, you may already be ready to introduce your first leveraged offer.

Often the smartest next step is simply packaging the process you already use with clients into a structured framework.

From there, you can deliver it in different formats.

If you're listening to this, chances are your business is currently built around one-to-one client sessions and your income, your schedule and your energy are tied to seeing clients one at a time. So I want to start with a question. How long do you honestly think you can keep pouring your time and energy into back-to-back one-to-one client sessions before something starts to give? Today's episode is all about how to know

if and when it's time to stop solely relying on one-to-one client sessions and exploring smarter ways to move beyond them so you can scale your business and your income whilst freeing up your time.

Now it's incredibly common for health professionals leaving the NHS to launch their private practice entirely around one-to-one sessions, as it usually feels like the safest and easiest and actually most familiar way to start. It makes sense, as it's what you know. In the NHS, your work was structured around one patient at a time. Sessions were scheduled. Your expertise was delivered directly and personally. And that...

mindset carries over completely naturally when you move into private practice. You've spent years being trained to provide care and guidance directly and the idea of delivering your expertise in any other way, well it can feel a bit unfamiliar. But, and it's a big but, what works in the NHS doesn't always translate perfectly into business. Because in the NHS your salary was set regardless of how many patients you saw

In your own private practice though, every session directly impacts on your income. Of course, most people don't start thinking about leverage or scaling when they first leave the NHS and they start their businesses. In those early stages, just getting clients and filling the diary feels like a priority. And that's completely understandable. But what often happens is that after a few months or a couple of years, the limits of the one-to-one model become

painfully obvious. You might be busy, you might even be booked up and feeling really quite accomplished. But if all of your income and growth and energy are tied to those one-to-one sessions, there's going to be a natural ceiling. But once you recognise that ceiling, you can start looking at smarter and more scalable ways to deliver your expertise, ways that let you grow your business without exhausting yourself or trading your time for money.

So with that in mind, let's explore the five biggest signs that it's probably time to start moving beyond one-to-one work and then how you can go about leveraging and scaling.

So sign one, the very first clear sign that it's time to move beyond one-to-one sessions is that your diary is full, but your income feels capped. So in other words, you're booked, you're busy, but your income only grows if you decide you're gonna work more hours. So to really pinpoint whether this is your situation, try asking yourself questions like, am I consistently booked every month, but still not hitting the amount of money that I would like to have?

in my business? Do I feel like there's no room left in my schedule to take on more clients without sacrificing things I like to do in my personal time or my energy levels? And if I wanted to double my income, would that mean doubling my client hours? So if you're nodding along to any of these, this is a really strong signal that your one-to-one model has reached its limit.

and it might be time to explore smarter ways to leverage your expertise.

sign to that it's time to move beyond the one-to-one model, you keep repeating the same advice. So if this is the case, it means your knowledge is systematizable. And what I mean by that is you can easily package up your expertise into a profitable course or a group program to help multiple clients at once. So ask yourself, do I notice myself explaining the same concepts to my one-to-one clients over and over?

and over again? Or do my clients keep asking the same kind of questions in our sessions? Or do my clients keep coming to me with the same challenges? If the answer is yes, that's a strong indicator that your one-to-one work contains patterns and patterns can be leveraged to create more scalable offerings.

Remember, anything you say more than a handful of times can be taught once. It can be reused many, many times and delivered without you actually being present every time. That's leverage there waiting to happen.

Sign three that it's time to move beyond one-to-one sessions is your clients need you less than they used to. Now this one surprises people, but when you're really good at your job, clients stabilize faster. They gain confidence sooner and they rely less on weekly hand holding. And that's not a reason to cling tighter to one-to-ones. It's actually a sign that your method works, that there's a big opportunity to package up your expertise differently.

for instance, through group programmes or online courses or membership modules, because your clients can then continue to make progress and achieve results without needing you for every step, while still having access to your guidance and support when needed. So this way, you can multiply your impact, you can help more people and you can reclaim your time without compromising the quality of the care that your clients receive. So ask yourself,

Are my clients completing their goals faster than I expected? Do I find myself checking in with clients less frequently? Or could I provide guidance that empowers them to take action independently rather than them needing me every step of the way? And when the answer is yes to these questions, it's a really clear signal that your one-to-one model has done its job and now it's time to multiply your impact beyond it.

Okay, step number four, you're turning people away because you're stretched too thin. So ask yourself, are you booked months ahead or running a waiting list? How many times have you had to say no or not now to a potential client because you're just too busy? Or how many times have you said yes to an inquiry, even though you know you're already maxed out, but you decided to just squeeze one more in just this once?

These are all really strong signals that your current one-to-one model is limiting both your impact and your income. And the fifth and final signal that it's time to move beyond one-to-one sessions is that your personal life is constantly bumping up against your clinic hours. And by the end of the day, you feel so exhausted. Family time, evenings and hobbies just get pushed aside because you've spent all of your energy in back-to-back client sessions.

So ask yourself, by the end of most days in my business, on a scale of one to 10, how much energy is left for anything else? How often per month are my evenings or weekends wiped out because I'm recovering from such a busy week? And do I feel guilty for missing family time or social events because I'm physically or mentally spent? So if you're nodding along to these questions,

then relying solely on the one-to-one model is taking too much from your energy and your life. So this brings us nicely into part two of this episode. What does moving beyond one-to-one sessions and leveraging your income actually look like? And how can you create new income streams without completely abandoning your one-to-ones? So the first thing to know is this isn't an all or nothing jump.

Moving beyond one-to-one sessions doesn't mean you have to ditch your one-to-one clients. The most sustainable thing that I recommend is keeping your premium one-to-one sessions while adding in what I call one leveraged layer. So I'll give you an example from my own business to help you visualize this. In my children's nutritionist business, my signature program called Fussy Eating Fixed was designed to help parents with their children's eating struggles.

But I didn't just offer one way for parents to work with me. I created three clear tiers, each offering different levels of support depending on what the parent needed and how much guidance they wanted. So the popular package was designed for parents who felt confident with a self-study approach. They got lifetime access to my Feasts course and the Fussy Eating Fixed Online Community. They're guided.

by the course content and the parent-led community, which allows them to move at their own pace. And this package works really well for people who want results, but they don't need direct expert support every step of the way, or they aren't ready to invest in a higher cost support package. The enhanced package, that's the next level up. So parents still get access to the course and the community, but they also have 12 months access to the feeding clinic.

And this was a private online group where they can ask me and my expert team questions. They could join live Q &A sessions. They could get extra guidance when they needed it. And this was perfect for parents who wanted a bit more reassurance and interaction with the experts while still being largely self-directed.

And then finally, the VIP package is the premium high touch option. Parents get everything in the enhanced package plus six weeks of personalized one-to-one support with me. And this starts with a two hour initial assessment where I carry out a full feeding, nutrition and sensory evaluation. We create a tailored personalized plan and provide them with a written report. And then we check back in at the end of the six weeks with a final 45 minute consultation.

but throughout the process, they have access to ask me through WhatsApp for any quick questions or get any support. And this is the package for parents who want a hands-on, fully supported experience. And because it's so personalized, spaces are very limited. Now, the beauty of this tiered system is that I'm still delivering one-to-one support, but I'm also leveraging my content, my expertise and my knowledge

so that more parents can benefit without me being booked for hours every single day. I've essentially created a pathway of support from self-study to high touch guidance so that every parent can get what they need and I can manage my time, my energy sustainably. this is the exact type of model I talk about when I say moving beyond one-to-one doesn't mean giving less. It means actually reaching more people.

There are tons of other ways you can structure your support so that you're not trading your time for money. And the benefits of doing this are endless because not only do you get your time back and protect your energy, but you also help to make your expertise way more accessible to way more people.

Think about it, if you had a clinic waiting room full every hour of the day, you wouldn't see them one by one forever, right? You'd redesign the system. Well, online is no different. So with all of that in mind, now is a really good time to just sit back and reflect on all of those potential signs and ask yourself the important questions. Questions like, am I at or near my income ceiling with one-to-ones?

Am I repeating myself every time I see a client? Does my business entirely rely on my availability? And would my life improve if I worked fewer client hours for the same income? Now, if you answered yes to even one of those, you're already at the point where leverage makes sense.
